Once you have registered your child for Summermusic, please take a moment to provide us with their medical and emergency contact information below. This form is also available in PDF form on the main Registration page.
When the form is successfully submitted, this screen will display the words “Message Sent” and a summary of the information you have provided.
Please note: We must be notified in advance if a child will be absent or late. We will call the parent numbers or the alternate if your child has an uninformed absence.
Deep River Summermusic 
Audrey will be visiting us in July. Richard or Catherine (Richardson) will be accompanying her for the sessions. We hope it is alright if the same adult does not come everyday as there is another younger child to take care of.
